Speak up in thirty seconds. One record from the photo to the action that fixed it.
For anyone on site who sees something wrong, and the EHS team that reviews it: close a concern, raise actions, or start an investigation from it. The link stays either way.

Where it connects
On the same loop: Incident Reporting, Incident Investigation, Ergonomics and 38 more modules in one workspace.
Method note
Near miss or concern? Plants draw the line in different places.
Some sites log anything that nearly happened as an incident. Others keep conditions and behaviours in their own stream, so the incident log stays about events.

One stream means nothing falls between two systems. Two streams keep the quick report quick, and keep the incident count from becoming a reason not to speak up. Both are defensible, and many sites change their minds as reporting matures.
So the reviewer decides what each concern becomes, and the link is kept.
- Close it hereClose the concern when nothing more is needed. The change is kept in its history.
- Raise actionsOwned and dated in Action Tracking. The concern shows how many are done.
- EscalateStraight to an investigation, which opens with the concern at the top.
Where Concern Reporting usually breaks
Four places speaking up goes quiet, and what the record does about each.
The reporter never hears back.
HSE’s guidance for major hazard leaders asks for a specific, prompt response to each concern raised, and for the person who spoke up to be told what action resulted.

Small events keep happening, and nobody joins them up.
The CSB found that smaller fires and explosions kept occurring at Imperial Sugar’s plants for years, without effective action, before the 2008 explosion at Port Wentworth.

Reporting takes longer than the job.
OSHA has warned that reporting rules which are unreasonable or burdensome, or enforced with discipline, discourage workers from reporting at all.

In UProcess Capture asks for the type and what you observed, with the place and a photo if you have one. Everything else waits for the reviewer.

HSE’s investigation guide says each risk control action needs a person and a timescale, and that checking implementation is the step most often overlooked.

In UProcess Actions raised from a concern live in Action Tracking with an owner and a due date. The concern shows how many are done, and an action can be set to close only after someone checks that it worked.

The record an auditor asks for.
Speaking up is only credible when the answer is on file.
- What was reported and when, with its photos and attachments.
- Who reviewed it and when the review started.
- When it was closed, and by whom.
- The actions or investigation it led to, with their status.
What’s in a concern
- Capture
- Type, what you observed, where, and photos or files.
- Review
- Under review and closed, each change kept in the concern’s history.
- Attachments
- Photos and files on the concern itself.
- Escalation
- To an investigation, which opens with “From concern”.
- Origins
- Raised from an inspection finding, a concern shows the finding and the round it came from.
Questions
Does every concern need an action?
No. A concern can be closed without any action.
Can a concern become an incident report later?
Not from the concern today. An investigation can start from it.
Can I attach photos and files?
Yes, to the concern itself. An investigation started from it uses them by reference, without copies.
Can a concern start an ergonomic assessment?
An ergonomic assessment can name the concern that prompted it, and shows it.
